I have two options for screening...
Option A
Full name, real phone number, and two references (references must be current within the past 12 months).
Option B
Full name, real phone number, and selfie with your driver’s license. You may cover up the address on your license. U.S. driver's licenses only. Military or workplace IDs will not be accepted.
Please keep in mind that once you screen, you'll never have to do it again. Additionally, booking forms are reviewed by me and me alone and your information will remain confidential.
For safety and privacy reasons, I no longer allow the use of cell phones during a session. Please be prepared to store your cell phone during your appointment. I do not require the phone to be powered off as you may receive an urgent call. I just ask that you do not take it out during our session.
